Fire Dept. Purchase Requirements

All Fire Dept. Purchase Orders are REQUIRED to have the following information:1. Tax Exempt ID # and/or Federal Tax ID#.
2. Fire Chiefs name and contact information including email address.
3. Billing address.
4. Shipping address.
5. All orders should be placed on Fire Dept. letterhead.
6. PO#.
7. All PO's must be faxed to: (408)-493-4444
